- April 2009 Regulation Update - Beginning June 30, 2010, all Branch 2 and/or 3 licensees are required to complete 2 hours of Integrated Pest Management as part of their renewal requirements

Provider forms & information
- Continuing Education Provider Form - This form must be submitted to become a registered provider with the Board. There is a $50 fee.
- Application for Continuing Education Activity - This application and the $25 fee must be submitted to have a course approved by the Board.
- Continuing Education Activity Roster - This roster sheet can be printed for attendees to sign. A copy of all rosters must be submitted to the Board for continuing education audit purposes.
- Continuing Education Certificate of Completion - This certificate can be used as the certificate of completion for the licensees. Students should retain continuing education certificates for at least 3 years
